Garnering Effective Telehealth 2 Help Optimize Multidisciplinary team Engagement

What is GET2HOME?

Children with complex chronic conditions who are discharged from the hospital to home commonly encounter problems and challenges, and 1 in 4 will readmitted to the hospital within 30 days. In our PCORI-funded randomized controlled trial we aim to determine the effectiveness of the GET2HOME intervention as compared to usual care on outcomes that matter to patients, families, and health systems.

The GET2HOME intervention includes:

  • a pre-discharge telehealth huddle on Microsoft Teams which allows families to speak with both the hospital team and their outpatient care team (physicians, home care nurses, pharmacists) before they leave the hospital (video on left below),

  • a discharge task tracker (shown at right) which allows the healthcare team and families in the hospital to know which tasks are completed and which tasks still need to be completed before discharge, and

  • a post-discharge telehealth huddle on Microsoft Teams which allows families to reconnect with the hospital team and their outpatient care team a few days after discharge (video on right below).
